The Rogosin Institute encourages dialysis patients to travel and visit family and friends. With adequate notice (usually at least one month) our social workers can help make hemodialysis travel arrangements for patients.
The Rogosin Institute dialysis units also accept transient and transfer dialysis patients with advance notice and approval of the administrator at the individual units. Acceptance is available on a space available basis. Therefore, it is best to plan your hemodialysis well in advance of your travel, especially during peak travel time.
Before a transient patient can be accepted for hemodialysis treatment, a packet of all updated medical history needs to be reviewed by the facility administrator at each dialysis center.
Please download forms, complete and fax back to the social worker at your dialysis center.
